According to the local safeguarding board regulations 2006, serious case reviews will be required in this situation where a child or young person had passed away due to known or a suspected abuse or neglect. Sometimes reviews may be carried out where a child or young person has been seriously harmed or has suffered some life threatening injuries. Serious case review is used for different agencies and they discuss the case together and what is to be learned about the way the professionals have worked and which they can work together in the future. A report will then be written and then will then be released to the public so the recommendations are unknown when undertaking a serious case review. The review is able to highlight where the mistakes were made.
